Losing baby teeth is a normal part of growing up—but timing matters.
When a baby tooth falls out naturally and the permanent tooth underneath is getting ready to erupt, there is usually little for parents to worry about. But when a baby tooth is lost months or even years earlier than expected because of decay, infection, injury, or another dental problem, we look at the situation differently.
One question we hear from parents is whether their child needs a space maintainer, sometimes called a dental spacer for kids. The purpose is fairly simple: a space maintainer helps keep neighboring teeth from moving into an area that may still be needed by a developing permanent tooth.
The American Dental Association explains that when a primary tooth is lost too early, surrounding teeth can shift into the opening and reduce the space available for the permanent tooth.
At Tebo Dental, we do not recommend a space maintainer simply because a baby tooth is missing. We consider which tooth was lost, your child’s age and stage of dental development, how the permanent teeth are developing, how much space is available, and several other factors before deciding whether maintaining that space would benefit your child.
What Is a Space Maintainer?
A space maintainer is a small dental appliance designed to preserve the opening left when a baby tooth is lost prematurely.
Unlike braces, which actively move teeth, a space maintainer is generally a passive appliance. Its job is to help keep neighboring teeth in their existing positions while the permanent tooth underneath continues to develop.
This matters because baby teeth do much more than help children chew until their adult teeth arrive. They also help preserve the shape and length of the dental arch and maintain space for developing permanent teeth.
A child may need that space preserved for several years. In other cases, the permanent tooth may already be close enough to erupt that an appliance provides little additional benefit. That is why an examination—and, when appropriate, dental X-rays—is important before deciding whether a space maintainer is needed.
Why Can Losing a Baby Tooth Too Early Be a Problem?
Teeth do not necessarily stay in the same position after a neighboring tooth is lost. Depending on a child’s stage of development, nearby teeth can tip, drift, or move toward the newly opened space.
The American Academy of Pediatric Dentistry notes that premature loss of primary teeth from causes such as decay, infection, trauma, ectopic eruption, or crowding can result in loss of arch length. When enough space is lost, it may contribute to crowding, rotated teeth, abnormal eruption patterns, crossbites, and other alignment concerns.
That does not mean a child who loses a baby tooth early is destined to need braces. It means we want to determine whether there is a meaningful risk of space loss that could make it more difficult for the permanent tooth to erupt properly.
Timing can also matter. Research examining premature loss of primary molars has found that some of the greatest changes in space can occur during the first few months after a tooth is lost. For that reason, we recommend having an unexpectedly lost baby molar evaluated rather than simply waiting several months until the child’s next routine appointment.
Does Every Child Who Loses a Baby Tooth Early Need a Space Maintainer?
No. This is probably the most important point for parents to understand.
Current AAPD guidance says space maintenance should be considered when a primary tooth is lost prematurely, but the decision depends on the individual child. The available research also does not support treating every prematurely lost baby tooth exactly the same way.

These considerations are consistent with factors identified by the AAPD when planning space maintenance, including the tooth lost, the amount of time since tooth loss, dental age, available space, development of the permanent successor, oral habits, oral hygiene, medical history, and the child’s ability to cooperate with treatment.
What Types of Space Maintainers Are Used for Children?
There is no single space maintainer that works for every child or every situation. The AAPD describes several types, including fixed appliances designed for a specific area, fixed appliances that span a larger portion of the dental arch, and removable appliances.
One common design is a band-and-loop space maintainer. A small metal band fits around a neighboring tooth, while a loop extends across the missing-tooth area to help preserve the space. Another variation is a crown-and-loop appliance, which uses a dental crown as part of the support for the space maintainer.
In certain younger children who lose a second baby molar before the first permanent molar has erupted, a dentist may consider a distal shoe space maintainer. This is a more specialized situation because the appliance helps guide the developing permanent molar as it erupts. It is used selectively and requires appropriate placement and ongoing monitoring.
When multiple teeth are involved, other appliances may be considered, including a lower lingual holding arch, Nance appliance, or transpalatal arch. Removable appliances may also be appropriate in selected cases.
The right design depends on your child’s individual needs, which teeth are missing, how much space is available, and where the permanent teeth are in their development.
How Long Does a Child Wear a Space Maintainer?
There is no standard number of months or years that applies to every child.
In most cases, we monitor the development and eruption of the permanent tooth that eventually needs the space. A space maintainer may remain in place until the permanent tooth is ready to erupt into the area.
AAPD guidance recommends periodically checking the appliance and monitoring the developing permanent tooth. As your child grows, a space maintainer may need to be adjusted, recemented, repaired, replaced, or eventually removed when the permanent tooth has erupted appropriately.
This is one reason routine dental checkups remain important after a space maintainer has been placed. We are not simply checking that the appliance is still attached. We are watching how your child’s entire mouth is developing around it.
Does a Space Maintainer Hurt?
A space maintainer is designed to hold space rather than actively push teeth into new positions. Your child may notice the appliance at first, particularly when talking, eating, or moving their tongue around it, but most children become more accustomed to it over time.
Parents should contact their dentist if the appliance becomes loose, breaks, causes ongoing discomfort, or begins irritating the gums or other soft tissues.
The AAPD identifies several potential complications that dentists monitor for, including appliance breakage, loss of cement, plaque buildup, cavities, soft-tissue irritation, unwanted tooth movement, and interference with eruption.
That is why we do not view a space maintainer as a treatment that can simply be placed and forgotten. Space maintainers can be very useful when properly selected and monitored, but regular follow-up is an important part of treatment.
How To Care for a Dental Spacer at Home?
Good brushing becomes especially important around a fixed space maintainer because food and plaque can collect around the band and neighboring teeth.
Younger children may need a parent’s help to clean the area thoroughly, even if they normally do some of their brushing independently.
Parents should also pay attention to anything that looks or feels different. If the appliance seems loose, a wire or loop has moved, your child reports pain, or a permanent tooth appears to be erupting around the appliance, contact your dentist rather than waiting for the next scheduled cleaning.
Your dentist may also provide specific instructions about foods that could pull on or damage the appliance. Following those recommendations and keeping the surrounding teeth clean can reduce the risk that the space maintainer itself contributes to a dental problem.
What If We Decide Not to Use a Space Maintainer?
Sometimes observation is the right choice.
If the permanent tooth is already close to erupting, there is plenty of room available, or the specific tooth loss presents a low risk for meaningful space loss, your dentist may recommend monitoring rather than placing an appliance.
In other situations, losing space could make it more difficult for the permanent tooth to erupt where it belongs.
The goal of a space maintainer is not to guarantee that a child will never need orthodontic treatment. Instead, it is to preserve existing space and reduce avoidable changes when the clinical situation suggests that the space is worth protecting.
We think that distinction is important. The AAPD recognizes that premature loss of baby teeth can reduce the space available for permanent teeth, while the available evidence does not establish that space maintainers prevent every future orthodontic problem.
Good pediatric dental care means evaluating the individual child rather than automatically placing an appliance after every early tooth loss.
What Should I Do If My Child Loses a Baby Tooth Early?
If your child loses a baby tooth that was already loose and clearly ready to fall out, that is usually a normal part of development.
If a baby tooth is knocked out, removed because of severe decay or infection, or falls out much earlier than expected, it is worth having your child’s mouth evaluated.
This is especially important when a baby molar is lost early because some primary molars are expected to remain in the mouth for many years.
At an evaluation, our team can examine the area, assess your child’s bite and available space, and use dental X-rays when appropriate to determine where the permanent tooth is in its development.
